    We understand that your child may have asthma. Asthma may be mild or severe, with many known or even unknown triggers. We want to know as much information about your child's type of asthma and triggers so we can provide the best help we can if an emergency were to occur.

    Montana State Law (20-5-420) allows capable students to carry and self-administer their rescue inhaler during school hours, before, after, and during any school activity, and anytime the student is on school property. In accordance with the law, appropriate paperwork is required for any student who plans to carry and self-administer their asthma medication.  If your child plans to carry their rescue inhaler at school, please complete the forms below.
