Travel Release- if your child is going to leave the game with someone other than a parent/guardian (grandparents included), this form needs to be filled out and signed by the building principal or athletic director prior to the game. If this form is not filled out in its' entirety, your child will not be allowed to leave with anyone other than their legal parent/guardian.
