•  MEDICATIONS AT SCHOOL:

    All prescription medication that must be taken by your child during the school day requires appropriate paperwork from your child’s health care provider. This paperwork is required prior to administration at Shepherd and needs to be filled out and signed by the quardian and health care provider. 

    **A new set of paperwork must be completed at the beginning of each school year. It cannot carry over from the previous school year.**

    Use this form for all medications to be administered at school, this includes medications that are prescribed and over the counter. The medication will be stored by the School Nurse in a secured location and administered by the School Nurse. 

    **Please note: OTC medication is not stocked or supplied by the school. School staff should not under any circumstances be providing any OTC medication to your child.

    Use these two forms for inhalers and Epi-Pens. The medication can be kept with your child if your child is capable and responsible to administer medication without the use of nursing or staff assistance. They will need to keep the medication in a safe but accessible place. If needed the medication can be stored in the Nurses office. The State of Montana requires an authorization to carry and self-adminsiter asthma or allergy medication form to be completed each school year. Please complete and return both forms.
